Assessment booking
Please select a convenient time for one of Virtue’s Advanced Nurse Practitioners to give you a call and complete a clinical history and assessment. Once you have selected the date and time, please complete the contact detail fields, and submit.
What happens after I submit the Referral?
Following submission of the booking you will be sent a calendar invite for the appointment. If for any reason you need to change the booking date/time there is an option to reschedule in the appointment email. One of the Virtue Advanced Nurse Practitioners (ANPs) will give you a call at the data/time you have provided. The calls last about 30 minutes and will involve you providing information related to your past medical and social history, medications, allergies, and any other relevant information to ensure Virtue has a good understanding of clinical background and needs. Please have your medication list available for the call and any other information you might find useful to support the ANP assessment.
